Social Worker
We are looking for an enthusiastic, driven, autonomous Social Worker to join our busy growing Rehab team! We are seeking a Social Worker who will be responsible to help our clients develop the skills and resources they need to enhance social functioning. As well the Social Worker will be required to provide counseling, therapy and referral to other supportive services where necessary. We are looking for assistance on a casual (as needed basis) for all of York Region. If you are looking for a rewarding career, look no further!
Responsibilities include (but are not limited to):
• Interview clients individually, in families, or in groups, to assess their situation and problems and determine the types of services required
• Provide counsel and therapy to assist clients in developing skills to deal with and resolve their
social and personal problems
• Serve as members on interdisciplinary teams of professionals working with client groups
• Plan programs of assistance for clients including referral to agencies that provide financial
assistance, legal aid, housing, medical treatment and other service.
• Document all assessment findings, treatment plans, progress and discharge summaries of
interventions as required by the profession, referral source and the agency
• The successful candidate must be able to make decisions and accept responsibility
• Provide education to patients, their families and team members
Qualifications:
• Bachelors’ degree in Social Work from an accredited University. Master’s degree is preferred
• Registered under the Social Work and Social Service Work Act, 1998
• Experience working with diverse, multicultural client groups
• Ability to work independently and co-operatively in a busy, multidisciplinary situation
• Excellent interpersonal skills and a demonstrated ability to build collaborative and therapeutic
interpersonal relationships are required- as are proven skills in successful teamwork,
communication and conflict resolution
• Ability to adapt to and work effectively within a variety of situations, and with various
individuals or groups. Demonstrated ability to be flexible
• Valid Ontario driver’s license and availability of a motor vehicle are required with proof of Auto
Insurance.
